There is a $25.00 non-refundable fee for this Application which you may submit via PayPal here. Once we have received the completed Application and fee, we will review the same and get back to you within a two-week time frame. Upon approval, you will be able to host the sanctioned USCSS™ trial applied for. We want all of our Affiliates trials to be successful, so we are happy to assign a mentor to help you along the way.
